At present, the 'Outcomes' Report available within Client Manager just lits the last Progression Note score that the client achieved.
This doesnt give any visibility of the journey the client has made towards being Independent .
Assisting the client to move from needing support.....to being independent again is the core function of Local Authority Reablement Services.
So, to make this more visible, we would like the Outcomes report to have two columns for Progression note scores instead of the current one.
There needs to be one column displaying the clients initial progression note score (against each outcome), and a second column showing what their final progression note score (against each outcome)

Business Case
The core purpose of all Local Authority Reablement Services is to support the client in a journey from needing support....to a point where they are more independent and Reablement support is no longer required. This idea will help LA Reablement Teams to evidence the journey towards independence the client has made. |
